How to Claim:
1. Customer must send this AH Beard 2 Fly Free Boarding Pass, completed Claim Form, a copy of the Sales Receipt/Proof of Purchase and a DL sized stamped (50c) Self-Addressed Envelope to:

2. All claims must be received within 30 days from date on Sales Receipt.
3. Within 28 days of receipt of valid claim, you will be posted a Flight Booking Form to book your flight.

*Terms and Conditions
1. The attached information on how to claim flights, the Boarding Pass, Claim Form and the Flight Booking Form form part of these Terms and Conditions. By making a claim, you confirm your acceptance of these Terms and Conditions. This offer cannot be used in conjunction with any other offer.
2. This offer is only open to persons who are Australian Residents aged 18 years and over. The offer is not open to Government or Educational departments. Employees of the Promoters, Suppliers and their Agencies (including Resellers and other Agencies associated with this offer) and their immediate Families are ineligible to participate. The following are eligible products, AH Beard FlexGel 1000, 2000 or 3000 mattress or ensemble in a Double, Queen or King Size, provided they are new and originate from Forty Winks stores.
3. This offer is restricted to one redemption per eligible AH Beard FlexGel 1000, 2000 or 3000 mattress or ensemble in a Double, Queen or King Size purchased in Australia during the Promotional Period (identified below). Only one return flight offer for 2 people may be reserved per booking.
4. The Promoters and the Suppliers reserve the right to verify the validity of all claims and disqualify any claimant for tampering with the claim process or for submitting a claim which is not in accordance with these Terms and Conditions. Illegible claims will not be accepted.
5. The offer opens on 22nd September 2008 and ends 31st December 2008 ("Promotional Period"). A copy of your eligible AH Beard FlexGel 1000, 2000 or 3000 mattress or ensemble in a Double, Queen or King Size products proof of purchase PLUS the original uniquely coded Boarding Pass PLUS a completed Flight Claim Form PLUS a business sized stamped self-addressed (50c stamp) envelope must be received by AH Beard 2 Fly Free, PO Box 657, North Sydney NSW 2060 within 30 days of the purchase date. Failure to provide a suitable Proof of Purchase, an original uniquely coded Boarding Pass, business sized stamped self-addressed envelope or incomplete Claim Forms will result in that claim being deemed invalid. Some fields are mandatory on the Claim Form and are required to validate your claim. Incomplete Claims will be deemed invalid and not be issued a Flight Booking Form or an additional opportunity to claim.
6. Your original Flight Booking Form must be completed in full and received by the travel centre at least 60 days prior to your requested outbound travel date(s). Failure to do so will make your claim invalid and no further flights will be offered and you will not be issued a further Flight Booking Form.
7. Approximately 21 days after the receipt of your original fully completed Flight Booking Form, the travel centre* (identified below) will contact you by phone regarding your flight request and flight availability. No amendments are possible after confirmation of your booking with the travel centre. Accordingly, claimants may wish to purchase travel insurance at the claimant's own expense. Ground and Air Taxes are included in the offer, however any ancillary costs will be charged to a valid credit card (merchant fees apply for credit card purchase) to confirm your Return Flight booking. Additionally, at this time, you may wish to take advantage of the travel centre's* preferred rates for additional flights, accommodation insurance and tours. You may also choose to buy insurance. (Merchant fees apply for credit card purchases).
8. Flights must be booked within 6 months and travelled within 12 months of the original eligible product purchase date.
9. The travel centre* will attempt to arrange flights on your selected preferred dates but no guarantee is made as to the promotional availability of seats or flights on such dates.
10. Acceptances are effective only when received by the travel centre*. No responsibility is accepted for late, lost or misdirected mail or claims. Proof of posting is not proof of receipt.
11. All flights are direct. No stopovers or alternate routes offered. You must depart from and return to the same point of departure.
12. All flights are arranged by the travel centre* and are strictly subject to promotional availability of flight times and dates for the flight requested. Flights cannot be taken during school holidays, public holidays, and special event periods. The Promoters and the suppliers in their absolute discretion will determine the exact flights provided and on which airline they are provided.
13. The Promoters and the Suppliers accept no responsibility for, and no refund will be provided, in the event that a claimant misses a flight. Additionally you will be charged the full retail cost of the flight plus an additional $50 administration fee should you fail to check in for your reserved flight.
14. In addition to these Terms and Conditions, all flights will be subject to the standard Terms and Conditions of ticketing and passenger transportation for the relevant airline.
15. Certain legislation, including the Australian Trade Practices Act 1974 (Cth), may imply warranties or conditions or impose obligations which cannot be excluded, restricted or modified except to a limited extent. If those statutory provisions apply, notwithstanding any other provision of these Terms and Conditions, to the maximum extent permitted by law, the Promoters' liability for any breach of those warranties, conditions or obligations is limited to, at the Promoters' option (i) supplying the services again; or (ii) paying you the cost of having the services supplied again.
16. Without limiting the operation of clause 14, and to the extent permitted by law, the Promoters are not liable for any loss or damage (including without limitation consequential loss or damage) or claim (whether in the form of an allegation, demand, suit, action or other proceeding of any kind) made in respect of the provision of the Promotion, Return Flights or otherwise under or in connection with these Terms and Conditions or their subject matter, whether arising directly or indirectly under contract (including under any indemnity), negligence or any other tort, under statute or otherwise at all.
17. Claims and flights offered under this promotional offer are not transferable or exchangeable and cannot be taken as cash. Only the person registered on this Flight Claim Form will be eligible for the Return Flight offer.
18. The Promoters and the Suppliers reserve the right to withdraw or replace this offer at any time with a product of similar value.
19. The Promoters and the Suppliers reserve the right to withdraw, cancel, vary or suspend this offer without notice in the event of war, earthquake, catastrophe, strikes or similar event.
20. The Promoters' and the Suppliers' decision in relation to all aspects of this promotional offer is final and no further correspondence will be entered into. All correspondence regarding flight arrangements should be directed to AH Beard 2 Fly Free, PO Box 657, North Sydney NSW 2060.
21. Independent of this offer and subject to additional conditions, the travel centre* will also be pleased to assist with accommodation and travel insurance requirements if required. Further airfares for accompanying travellers can be purchased from the travel centre at market prices. There is no guarantee as to the availability of seats on the same flight, and all additional flights are subject to availability and conditions apply.
22. Your personal information is being collected by the Promoters. It will be used for purposes relating to your participation in the Promotion, including booking your flight and contacting you about your flight. In administering the Promotion, your personal details may also be disclosed to third party service providers 23. The Promoters are AH Beard Pty Ltd (ABN 47000060677 of 38-64 Bryant Street, Padstow NSW 2211. The Suppliers are Leapfrog Promotion & Incentive Solutions Pty Ltd (ABN 58 097 950 108) of Level 16, 16 Lavender Street, Milsons Point, NSW 2061. *The travel centre is Express Touring Pty Ltd , (ABN 25 117 473 373) of Level 16, 61 Lavender Street, Milsons Point NSW 2061.
